Donation form

This form is for physicians and hospitals to carefully register a (potential) donation procedure after death. It provides structure for reporting and helps ensure the correct steps in the donation process are followed.

Examination envelope for extramural

The envelope contains the most important documents for a tissue donation procedure. One set contains 10 envelopes. Add the A and B forms yourself so that you have all examination papers at hand. The envelopes cannot be downloaded digitally, but can be ordered for free here.
