Materials for the donation procedure
Donation form
This form is for physicians and hospitals to carefully register a (potential) donation procedure after death. It provides structure for reporting and helps ensure the correct steps are taken in the donation process.

There are two brochures available for loved ones of a (potential) organ or tissue donor.
